Today, I watched another patient hug my physical therapist goodbye and cry. He’s leaving the practice, and because he’s so good, many of us are disappointed. I certainly am.

We talk a lot about finding a good provider, but if you are chronically ill losing a provider is a scenario you’re also bound to face, and we need to talk about it.

How you manage the transition may determine whether you continue to move forward—or lose ground.

The key is to use your final visits strategically; they are important opportunities most patients don’t realize they have. Today I’m explaining exactly how I approach mine.

The requests I make in those final visits ensure I:

  • Find the right replacement

  • Better understand my own health

  • Optimize critical medical documentation
